Medication Refill Request Form

IMPORTANT: After you have submitted your request online, via email, or by phone, the reception team will notify the doctor of your refill request and will then contact you either when the prescription is filled and ready for pickup, or if your pet is not eligible to receive your request. If you submit a request online or by leaving us a voicemail, you will not be contacted by a team member until your prescription is ready for pickup, or if there are any issues that prevent us from fulfilling the request.

Processing may take 3-5 business days for prescription refills  and 7 days for all compounded medications requests.

It is required that all pets must have had an examination within the past year (or within 3-6 months in certain cases) for a veterinarian to be able to prescribe any medications. This process guarantees the safety and ongoing assessment of your pet's health during the use of these prescriptions. However, over-the-counter products do not necessitate an annual physical examination.

 New Prescription Protocol:
Starting October 28th, 2025, a $20 rush fee will apply to all prescriptions requested to be filled and picked up on the same day. In addition, a $20 fee will be charged for any written prescriptions that clients request to be sent directly to the pharmacy, provided the hospital is able to fulfill the prescription. These fees are being introduced to better manage the increased demand on our pharmacy services.
